Form	目的
Academic Reduced Course Load Form	Federal regulations pertaining to 的 enrollment of F-1 students in 的 U.S. 需要 that a student be registered for and complete a full course of study during academic 条款. Exceptions to 的 full-time enrollment 需要ment are limited by regulations and include specific academic difficulties.  A DSO may authorize a reduced course load for a student experiencing academic difficulties, but only on 的 basis of 的 following reasons specified in 的 regulations. This exception may be made only once per academic level. Additionally, you must still maintain at least 6 credit hours during 的 given term.
Curricular Practical Training (CPT) Application Form	Curricular Practical Training (CPT) or "internship", is a temporary authorization for practical training directly related to 的 major field of study (as listed on 你的I-20). Practical training can include employment, internship experience (paid or unpaid), and education experience. CPT is authorized by 的 International Student 服务 Office in accordance with F-1 regulations. After you have completed all of 的 CPT Application Steps, please complete 的 final application to be considered for CPT.
F-1 Advising Form	All F-1 College of Business Graduate Students are 需要d to complete 的 F-1 Advising Form prior to registration. You will not be able to register for classes if you have not submitted 的 form. After you have submitted 的 form, and it has been reviewed by your Academic Advisor, you will be notified via email when you are clear to register for your classes.
F-1 Letter Request Form	Please use this form to request an official letter from 的 International Student 服务 Office for items such as Enrollment Verification, Social Security Numbers, and Family Visa Invitations for graduation.
Medical Reduced Course Load Form	Federal regulations pertaining to 的 enrollment of F-1 students in 的 U.S. 需要 that a student be registered for and complete a full course of study during academic 条款. Exceptions to 的 full-time enrollment 需要ment are limited by regulations and include medical incapacity. Please use this form to formally request a Medical Reduced Course Load.
New F-1 Student Check-In Form	This form should be completed by admitted students prior to F-1 Student Orientation.  You will be 需要d to upload a copy of your most recent I-94 and F-1 visa or I-797. After your documents have been reviewed, and you have attended F-1 Student Orientation, 的 International Student 服务 Office will provide you with a new signed I-20.
OPT/STEM OPT Application Review Form	Optional Practical Training (OPT) is work authorization granted by 的 Department of 首页land Security. Immigration regulations allow F-1 students to apply for an initial 12 months of OPT to gain practical experience related to 的 student's field of study (academic major). OPT can be authorized at each academic level, e.g. 12个月 的 bachelor's level, 12个月 的 master's level, 12个月 的 doctoral level. If you have completed a STEM designated degree, you are also eligible to apply for a 24 month STEM OPT Extension at 的 conclusion of your initial 12 months of OPT. Please complete 的 form officially request your OPT Recommended I-20 for your initial 12 months of OPT or your 24 month OPT STEM Extension.
OPT Reporting Form	All University of Dallas F-1 students, who are currently on OPT/STEM OPT, are 需要d to report any changes in name, address, employment, etc. within 10 days of making 这些变化. STEM OPT students should use this form for 的ir six month check-in and annual evaluations.
SEVIS Record Transfer Request Form	Please complete this form is you would like your F-1 Student SEVIS record to be transferred to a new school. Please note that you must be admitted to 的 new school before your SEVIS record can be transferred.
